I just went through all the score mode levels on Nightmare and i figured id share the info i learned:

1.) Of course most know you dont want to get hit it lowers your multiplier number, but whatever you dont dont die because it resets it all the way back to 0!

2.) All of the Giant Chests, gates, and dual gates count towards your portal master stats, so open them all. If you got through the game the first time you will realize you are still far from opening all the gates and chests you need.

3.) Use a weak character, points seem to be based on number of times you hit, not how hard you hit. So the longer it takes you to kill enemies the more points you are getting.

4.) Take time killing enemies that are spawned by chompy pods, undead spell punks (phantasm forest) etc. more enemies means more points.

5.) Go into all the Swap zones, and gate zones etc, the reward will be a +1 to your multiplier, you can have a maximum of 30 on your multiplier.

6.) Playing in nightmare gives your more points, but it does not count towards your completion of nightmare levels. You can only get those playing story mode in nightmare.

7.) Any of the spots where you shoot targets with a cannon will also give major points, especially if you complete them with a "perfect" Another thing is if there are bombs to shoot you can collect points and then shoot the bomb to reset the game and keep collecting points. For example in the Frostfest Mountain level there is a cannon game.
