Does anyone know exactly how to obtain this thing? I seem to randomly get it when I'm not trying, and vice versa when I am trying.

I've tried everywhere to find out, and not even Activision can answer (?!).

I'm starting to believe its defeat every enemy or discover every enemy in that chapter. In fact I think its discover every enemy because do to a elemental gate I couldn't discover all the areas nor all the enemies, in certain chapters.

It's kill. I solved this problem after skipping over an enemy.

Ah, okay. I must be missing an enemy somewhere in Oilspill Island - It's the only place I can't seem to complete now.

I'm currently stuck at this too, I think you are missing the same enemies that I am in the Fire Elemental gate section. There are three trolls on a platform in the center of the place and I can't seem to get to them. Has anyone reached them yet? I've been entering pipes like crazy but I can't seem to find the right pipe that leads me to them...

There are loops in those pipes so you have to be methodical and be sure to try every pipe on every island.

It's kill every enemy.

But some levels (later on) screwed up and didn't give me it.
If that ever happens, just repeatidly kill enemies from a respawner (dunno why this happens).

This happened to me on Molekin Mine and Quicksilver Vault.

I think it's a certain "kill" number that you need per stage?
I found in certain stages, if there were certain types of enemies that would create or make smaller enemies... I would not kill the big ones just their babies or mini's. Let them throw there little projectiles enemies at me....I would just sit there and kill those for awhile.
Then I would get the enemy goal.

When trying to get the "All Enemies" check for Oilspill island (Chapter 5), there are two things I did to successfully complete it, besides killing all of the enemies I came across during the normal course of gameplay.

The first one happened past the Fire elemental gate in all the pipes and platforms. There is a platform that shows three enemies that I could not figure out how to get to. There is a second platform that is close to the mystery platform that shows the three enemies on the right of the screen. On that screen, place an Adventure Pack peice on the portal (I used the Ice Mountain from "Empre of Ice.") Be sure to use the item itself, not the character or the two smaller magic items. Doing so will result in icicles falling from the sky on top of the three enemies on the mystery platform, destroying them.

Second, after you have rescued all five Gurlgefins, be sure to destroy all of the enemies in the center pit before throwing the bomb in the hole. They come in 3-4 waves. You can either throw the bomb in the pit, or use Eruptor to bounce his fireballs over the rail.

After doing these two additional tasks, I finally got the "Enemy Goal" check, and the last gold star!

Still having this issue in Quicksilver Vault. I don't think any of the enemies spawn minions in this level. Been through it with all of my characters, but still not complete. I am told that the only way to complete this level is with Sonic Boom. Goin to keep trying until I get my hands on a Sonic Boom, but totally aggravating until I do..... Currently at 99% complete, and only need this to complete the game!

I think Double Trouble's clones count too don't they? When they die?

I used one of Flameslinger's upgrades where he shoots 3 arrows in the air

Yes they do.

I am missing this in my completion, on the Quicksilver Vault level. I have read that Sonic Boom is required to achieve this. I have been hawking the stores for almost two weeks, but haven't been able to find a Sonic Boom. This is SO aggravating! I am even looking on ebay, but the Sonic Boom characters are going for $20+ shipping there. Gonna hold off for now, but this is really pissing me off!

I got sonic boom just by mistake. When ordered zap on eBay, I got sonic boom instead. He emailed me a week later and asked if I would like a refund. I reply saying that I would have more fun with sonic boom then zap, so it all worked out.
