I'm right before the final boss and I think so far, it's easy enough if you know what you're doing. I only started spamming Sky-Chi attacks in the abandoned amusement park because of that battle after frosting the cake. They come in handy in the Imaginator fights too.
At the moment I'm trying to get the time bonus for each level with no lives lost. I have all levels done except the dragon temple. I died in the Bowslinger fight and wasn't in the mood to go through 10 minutes of stuff I did already.
Long story short, I find it disappointing how literally nothing changes except enemies' health and attack power. Even Trap Team had some changes in Nightmare.

Honestly I didn't notice much of a difference from expert to nightmare. I practically never use sky-chi either, I've used it less than a dozen times across both playthroughs. I usually forget that it even exists. I was disappointed that the Kaos fight wasn't at least switched up a bit.

Are you trying to do the speedrun AND no lives lost objectives at the same time?

You can do 1 of the 2 objectives in one run, then the other objective in another run.
