This may end up being an easy answer - so I'll start with the first question:

How do you unlock Nightmare?

2nd question (if applicable) -- I'm starting on medium, should I then go through the game on Hard mode or Nightmare mode. (would you answer differently for an adult vs. a kid)?

1. You unlock it by beating Super Evil Kaos (final boss) on any difficulty.

2. You should try Hard mode first, then switch over to Nightmare. On Hars mode, enemies do more than 100 damage per hit, and on Nightmare, some do over 300.

Will still be checking the forums every now and then!

Just one question. Unlike Giants, are we able to switch from Nightmare back to any other mode easier than that in Swap Force or do you remain stuck in Nightmare Mode for eternity?

You can switch. I played Nightmare on my original profile for Swap Force.

1. Clear through the main story on any difficulty.

2. I would go through on Nightmare for the unlockable.

Nightmare difficulty is recommended to help increase your portal master rank. Also the Volcano Hat is totally worth the effort in my opinion.

I did Hard Mode in my initial playthrough, and Nightmare thereafter. You could go from Medium to Nightmare, but I highly suggest leveling your characters at least to 15, investing in a hat with speed and health bonuses, and unlocking as many charms as possible. Good luck!

Come to think of it, some of the new hats in Swap Force now provide health boost, including Legendary Treasures and Charms. Also, I suggest using Skylanders that deliver heavy amounts of damage and/or that have attacks that hinder enemies from attacking you. I would also recommend using Magic Items and Adventure Pack figures while struggling in Nightmare Mode.
