I know this is a Jojo thing, but what is the context of this? Why is it important there are 4 testicles? Does this have any bearing on the plot??????

The context is that Josuke came out of the ground naked and this girl saw his junk

Yes it does have bearing on the plot, though it's a spoiler so I'll put it in a spoiler tag in case you really wanna know

The area of ground Josuke was buried under has a strange property where burying two things causes them to switch parts. Josuke was found buried here with no memories, and later the corpse of a man named Yoshikage Kira was found there, and also he had no testicles. So Josuke is actually a combination of Yoshikage Kira and somebody else, but with a bunch of parts swapped. Josuke also has half and half eyes and tongue because of this. I'm not completely caught up yet so I don't know if any more is known to this.
