I was given this form by the school psychologist because I'm a SPED, and I thought it was pretty interesting. It's 43 questions, and the questions are rated on a 1-5 scale. 1 being never/not true at all, 2 being rarely/not always true, 3 being sometimes/sometimes true, 4 being often/usually true, and 5 being always/completely true. It really doesn't actually matter how you answer the questions. If you don't like the 1-5 scale, feel free to answer them in your own way.

1. Forgets to turn in work.
2. Is perfect in every way.
3. Unable to sit down for a long time without moving (tapping your foot/shaking your leg).
4. Is one of the last to be picked for teams or games.
5. Restless or overactive.
6. Does not know how to make friends.
7. Runs/climbs when he/she is not supposed to.
8. Cannot grasp arithmatic.
9. Is difficult to amuse/serious all the time.
10. Needs extra explanation of instructions.
11. Is hard to motivate (even with rewards like candy or money).
12. Makes mistakes.
13. Acts as if driven by a motor.
14. Starts fights with others on purpose.
15. Has trouble getting started on tasks or projects.
16. Is happy, cheerful, and has a positive attitude.
