Technical problem with unlocking nightmare avenger achievement [CLOSED]

Has anyone else run into this problem. I have beaten the game on nightmare mode 100% and nightmare avenger/knight helm hat will not unlock. I've completed 43 other achievements with no problems. I've tried beating chapter 16 over and over. I called Activision and all I was told was to clean the cache and to do a hard reboot. I have done all that they have asked and the problem persists. Anyone ever had this problem and fix it? Maybe I missing something. Please help.

Oh and I play on Xbox 360. I can't be the only person to have this problem : (
